- 30% off periodontal treatment needs
- 15% off minor services, such as, but not limited to, dental sealants, fillings, or extractions

Program Guidelines
- Patient’s portion of the bill is due the day of the service
- There will be a $50 reinstatement fee if your plan lapses
- Cannot be used with any other plan or insurance
- No refunds of membership fee
The Trailhead Guarantee Program
An exciting benefit to joining the Trailhead Advanced Dentistry Membership Savings family is The Trailhead Guarantee. Dental work performed in our office is performed at the highest level. We believe it should also come with the greatest of peace of mind. This program warranties any of the work listed below for the stated time period. The only thing you have to do is come in for your regularly scheduled cleanings and exams. This is so we can ensure that your oral health stays optimal and we can catch any small issues before they become big ones.

**Terms apply: Warranty is voided if patient fails to keep regularly scheduled appointments for cleanings and exams every 3-6 months as required for your level of gum health. Warranty covers failure in materials or workmanship, but not usual wear and tear or damage caused by negligence. If the above terms are met, we fix it NO QUESTIONS ASKED!
